This document contains templates for a written commitment of manpower, equipment, and materials and for an oil spill contingency plan, which are appendices for professional engineer (PE)-certified, Tier I, and Tier II Spill Prevention, Control, and Countermeasure (SPCC) Plans. These appendices are required for facilities that have determined that required secondary containment is impracticable and have not submitted a Facility Response Plan (FRP); facilities with qualified oil-filled operational equipment that does not have secondary containment; and onshore oil production facilities that do not provide secondary containment for flowlines and intrafacility gathering lines.
The EPA requires that certain facilities develop and implement oil spill prevention, control, and countermeasure (SPCC) plans to help avert oil spills from reaching surface waters. This requirement applies to non-transportation-related facilities with a total aboveground oil storage capacity greater than 1,320 gallons (gal) or total completely buried oil storage capacity of greater than 42,000 gal. It applies specifically to a facility’s storage capacity, regardless of whether the tank is completely filled. In addition to the storage capacity criterion, a facility is regulated if, because of its location, the facility could reasonably be expected to discharge oil into navigable waters of the United States or adjoining shorelines. Owners or operators of onshore oil drilling and workover facilities have additional requirements for their SPCC plans. This plan template is designed for such facilities.
This checklist covers the areas of the Spill Prevention, Control, and Countermeasure (SPCC) Plan and facilities that EPA inspectors will review during an SPCC facility inspection and addresses the requirements for onshore oil drilling and workover facilities. This checklist should be consulted annually to review all Plan components and their implementation.
To comply with integrity inspection requirements for bulk storage containers, each shop-built aboveground bulk storage container is inspected/testing on a regular schedule in accordance with a recognized container inspection standard based on the minimum requirements in this table.
This form may be included as part of your facility's SPCC Plan to document dike discharge events.
The information in this form must be provided to the National Response Center (NRC) in the event of a discharge of oil to navigable waters or adjoining shorelines from onshore oil drilling and workover facilities.
This form must be included with your facility's SPCC Plan to document 5-year Plan reviews and any technical amendments made to the Plan.
Include this form with your SPCC Plan to keep a record of all inspections of aboveground bulk storage containers and piping.

An Emergency Action Plan (EAP) is required at some facilities and strongly recommended for all others. An employer (property owner or occupier) must have an EAP whenever a specific OSHA rule requires one. For example, an EAP is required at any facility where employees are required to evacuate when a fire alarm is sounded. Employers at facilities with fixed extinguishing systems and fire detection systems must also develop an EAP.
